The Trans Map project builds on the success of a series of nine Transition Information Network regional seminars held last year. The seminars brought together key decision-makers from all local authorities and their partner agencies to examine current good practice and challenges. The mapping exercise started with visits to partners involved in regional seminars and included a review of actions taken since and ways to further support developing plans.
CDC transition officer Sara Merriman carried out most of these visits. Sara explains: "There is a lot of innovative good practice out there and we need to share it more widely. But the reccurring problem is that different entitlement criteria and definitions between child and adult services often leads to a lack of continuity in provision across this important transition."
