NCB Now: CDC set to lead on transition support

The Department for Children, Schools and Families has named the Council for Disabled Children (CDC) to act as the national transition support team for the Aiming High for Disabled Children transition support programme.

During the transition to adulthood, disabled young people can face more challenges than their non-disabled peers, and may require support to access further education, housing, employment and leisure services.

Led by CDC, the transition support team will work with disabled young people, families, government, local authorities and primary care trusts.
