NCB Now: Transition moves up the agenda

Aiming High for Disabled Children placed disabled children high on the national agenda. Now the Transition Support Programme is helping to make a difference locally, with the support of the National Transition Support Team.

The Transition Support Programme (TSP) was developed as part of the Aiming High for Disabled Children programme. It seeks to ensure that transition support for disabled young people and their families meets minimum standards and provides opportunities for all local areas to extend their practice.

The National Transition Support Team (NTST) supports the programme by co-ordinating the work with local authorities, primary care trusts and regional advisers across England. NTST is a coalition led by the Council for Disabled Children and was launched in December 2008 by schools minister Sarah McCarthy-Fry. The coalition has three roles: to drive and shape the programme; to promote good practice and develop learning; and to identify the support needs of local areas. One objective is to develop practice around advocacy and the participation of disabled children.
