NCB Now: TransMap highlights best practice

The project builds on the good practice gathered for the Transition Guide for All Services, produced by the Council for Disabled Children, and the success of a series of Transition Information Network seminars that took place in 2007/08. The mapping work examined practice across a number of areas in England. It looked specifically for practice that has a direct effect on outcomes and that illustrates co-operation across services and the involvement of disabled young people and their families.

The TransMap work identified five principles that local areas should implement to ensure that young people regularly have a positive experience of transition. The TransMap report, which will be available shortly, examines ways the five principles can be implemented. Alongside each principle are examples of effective practice and service development, and it will be of use to all local areas who are trying to develop their transition processes. The principles, based on the needs and aspirations of young people, can also ensure a cohesive approach to service provision.
