The three-year review looked at the needs of young people from the first-aid organisation's Badgers, Cadets and 16-25 age groups through questionnaires, focus groups and discussions. Staffing changes at county level may also occur.
Sarah Holmes, chief commissioner for youth at St John Ambulance, said: "The Cadets programme has been going for 82 years and has never been reviewed. We attract a lot of young people but we don't always manage to keep them."
The organisation currently has 8,000 Badgers aged five to 10, 12,300 Cadets aged 10 to 18 and 5,400 18- to 25-year-olds.
Changes include a rebranding of Badgers' uniforms and Cadet badges and a new Amalfi Challenge for 16- to 25-year-olds.
