- As one of 13 transition practice development sites, what are you focusing on?

Mainly developing a multi-agency single assessment process for young people in transition. As well as engaging with a number of agencies to introduce this process, we are using the opportunity to develop a multi-agency transition protocol, which will outline the responsibilities of each agency, incorporating the assessment.

We are also developing a mini-enterprise project and an accessible transition website, and looking at how to address the needs of young people with Asperger's syndrome.

- What are your greatest challenges and how have you overcome them?

Ensuring that the many agencies that play a part in transition are engaged and stay engaged with developments. Transition will not be successful without this engagement, which has meant ensuring senior level professionals sign up to projects. Various agencies, such as social care services, health services, Connexions, the voluntary sector and our own advocacy service, are invited to participate in our development groups, and commitment from these services to participate is actively being sought.
