During an inspection of youth courts, findings of which were published earlier this week, HM Inspectorate of Court Administration found widespread confusion about who was responsible for young people after sentence.
The report says this "has resulted in vulnerable young people being left unaccompanied and in unsuitable accommodation".
Local authority children's homes generally hold the youngest and most at-risk young people who have been sentenced or remanded. The inspectorate has also raised concerns about safety and security.
Chief inspector Eddie Bloomfield said: "Overall, youth court culture has changed over the past five years."
