The two councils beat off competition from six other short- listed local authorities to win the grants, which are also sponsored by radio station Capital 95.8.
Hounslow will spend its money on a 12-week scheme called "Cheer up Hounslow", which will focus on coaching young people with disabilities.
Lambeth will use its cash to set up the Lambeth Community Judo Club, which has already received the backing of the British Judo Association.
The other London boroughs shortlisted for the 5,000 awards were: Bromley, City of London, Haringey, Hillingdon, Royal Borough of Kingston and Tower Hamlets. Every London borough has been awarded 1,000 from the programme, which will boost their preparations for the London Youth Games 2007.
