Smooth Moves is designed to bridge the gap parents may feel is left by their child moving from a children's centre to a primary school.
It also provides workshops to train childcare staff to support parents whose children are moving on to secondary school.
Gailyn Groves, development manager for parental support and family learning at ContinYou, said Smooth Moves aimed to fill a hole in assistance.
"There are some programmes out there but they're aimed at children who get to visit their new school and meet the pupils," she says. "There aren't many packages out there to support parents, yet they may have anxiety about their child moving on."
