Help From: The Transition - Information Network

The network is an alliance of organisations aiming to improve the experiences of disabled children during the transition to adulthood.

It offers advice and information to children, their families, carers and professionals on policy change and how it affects them.

Its magazine, My Future Choices, is published three times a year and contains real-life experiences of disabled young people, their difficulties and achievements. The network's web site lists events, clubs and organisations.

The network moved to The Council for Disabled Children this week when its funding from The Diana, Princess of Wales Memorial Fund ended.
