The recommendations were made by clinical leads NHS Connecting for Health, the IT programme responsible for the implementation of electronic patient records.
The team has produced a discussion document and wants to exclude "potentially sensitive information" from records that can be seen by health professionals without patient consent such as doctors in accident and emergency units and on out-of-hours visits.
"Sharing information isn't always an advantage and excluding details will improve confidence among children," said Viv Crouch, a schools nurse and lead nurse for the Teenage Pregnancy Partnership Board.
A final version of The Clinical Development of the NHS Care Record Service is due on 31 July. Records will be rolled out in stages from next year.
