In the first audit of child protection procedures in NHS Wales, the commission found most organisations checked new recruits with the Criminal Records Bureau but not all had rechecked permanent staff or checked temporary staff or volunteers.
In addition, many staff had not been trained in child protection and record-keeping systems needed improvement.
Each organisation submitted three areas for action, but most did not mention criminal records checks or record keeping. The most frequent areas listed were staff training in child protection, clinical performance monitoring and cross-organisation working.
Liz Fradd, the commission's director of nursing and lead for its children's programme, said: "The fact that it isn't in their list of three doesn't mean they are not going to do something about it.
