When a young person who is involved with Connexions reaches 18, they will be invited to a meeting with their personal adviser and a Jobcentre Plus personal adviser.
They will discuss the young person's needs and work out the best way for him or her to move forward into employment, training or further education.
The scheme is voluntary and if a young person wants to continue to receive support from Connexions, the handover can be carried out gradually.
Martin Vowles, area manager for Connexions, said the scheme would enable more people to benefit from Jobcentre Plus.
"It can offer a lot of support, particularly financial help, that we cannot," he said.
Vowles said there were no problems with confidentiality because each young person would be involved throughout the handover.
"We would not do it without the young person being part of the process," he insisted.
