A: A pathway plan is an agreement made between a local authority and a young person who is living in care about how the local authority will support and provide for the young person in the future. It should include plans for the young person's education, training or employment, dates for leaving care and where they will live once they have left care.
The pathway plan should be written as soon as possible after an assessment of the young person's needs is completed. Such an assessment is ordinarily carried out once a child reaches 16.
