In a report released last week, Anne Owers said Huntercombe YOI, in Oxfordshire, has made improvements, but the design of its units - each of which holds 60 juveniles - made it hard to run the establishment safely.
This led to high use of force, including strip-searching. Owers described this as "unacceptable".
