The NHS Early Years LifeCheck and NHS Teen LifeCheck are designed to help services identify health risks among babies and young teenagers and advise on goals and support that could reduce those risks.
The early years tool will provide advice to parents with babies aged around six months while the one for teenagers will cater for the needs of 12- to 15-year-olds.
The Department of Health intends to introduce the tools in 83 local authority areas in England and hopes the tools will be used by a wide range of services including primary care trusts, local authorities, schools, voluntary groups and GPs.
The LifeCheck programme was originally announced in the Our Health, Our Care, Our Say white paper, which was published in 2006. The programme aims to provide methods of checking people's health at key stages in their life.
