A Conference Focusing on Transition to Adulthood for Disabled Young People.

The Transition in Action group, of which Children in Wales is a founding member, is organising a conference which aims to improve future planning and provision of transition services in Wales. This includes looking at transition planning within schools, transition to further and higher education and employment.

Disabled young people will lead the morning session suggesting ways in which the transition process can be improved. The afternoon session will include workshops showcasing examples of good practice.
