The 52-year-old man was jailed in August 2003 for 15 years for incest and sexual abuse. Care workers had visited the family since the 1970s but suspicions were not acted on until 2002.
In its case review published last week, Swindon Area Child Protection Committee called for the definition of parental responsibility to be updated and for a protocol on how to respond to anonymous allegations.
The committee also recommended a working party to implement information sharing.
