Social Care News: Child protection - Call for system to record'near misses'

Children would be better protected if a national system for learning from near misses, not just tragedies, was implemented, the Social Care Institute for Excellence said this week.

The institute made the recommendation in a report on managing risk andminimising mis-takes in children and families services. In the report itsuggests bringing social care in line with the National Health Service,where learning from adverse incidents and near misses is co-ordinated bythe National Patient Safety Agency, which the Government set up in July2001. The agency looks at the root causes of incidents, rather thanidentifying individual failure.

"We're encouraged by people starting to think in this way, we think thereport is going in the right direction," said Ian Johnston, director ofthe British Association of Social Workers.

Other recommendations in the report include piloting critical incidentreporting within children's services and exploring how to promote ablame-free approach to learning.
