Serious Case Review: Missed opportunities to end father's abuse

A 15-year-old from West Sussex who was known to social services became pregnant after she was raped by her father. Joe Lepper reports.

Two years ago, Mary, a pregnant 15-year-old, struck up the courage to tell her GP that her father was also the father of her child.

Following a police investigation it emerged that the father, who is now serving a life sentence for rape, had subjected Mary and her five siblings to years of abuse.

Social workers were among a number of professionals to let the family down, according to a serious case review (SCR) by West Sussex Safeguarding Children Board.

The SCR found that social services had been involved with the family since 2002, when the mother became mentally ill. But an unqualified assistant social worker was assigned to the case.
