
After taking a cocktail of drink and drugs at a flat in Newquay, homeless 17-year-old Lorna May fell unconscious, choked on her vomit and died in March 2009.
A serious case review (SCR) into her death by Cornwall and Scilly Isles Safeguarding Children Board revealed how just six months previously she had followed her homeless mother onto the streets and how social workers had failed to support her.
The SCR identified a "silo mentality" among Cornwall County Council social workers and other professions across housing and health. The review found that professionals were more concerned with "the performance indicators and resources, as opposed to focusing on the needs of the family".
