Hampshire County Council's Reachout campaign has recently been awarded a good practice award from The Prince's Trust for its work in providing mentor support to young people leaving care. For those who don't have the support of parents and wider family members, learning to live independently can be very difficult, but with more than 40 volunteer mentors, Reachout offers support, friendship and advice on future learning for young people aged 16 to 21 who are leaving care.
