Lewisham council in south London is linking agencies including education, social services, health, police, Connexions and its Youth Offending Team.
A central team of four people will run the operation, supported by professional advisers from education, social services, health and housing.
Access to the web-based system will only be allowed after staff have undergone full training. They will also be trained on the "concept of information sharing", said David Webb, Lewisham's head of performance, quality and special projects.
He said the council and local health agencies would provide the bulk of the 500,000 needed to run the system for 18 months.
