Transition: getting it right for young people, jointly published by the Department for Education and Skills and the Department of Health, calls for voluntary youth organisations to disseminate examples of good and bad practice and educate health professionals, as one of nine action points. It also calls for more partnerships between Connexions and youth services and health services, and the development of a transition training resource drawing on the expertise of groups such as the National Children's Bureau (NCB).
The report says: "Poorly planned transition from young people's to adult-oriented health services can be associated with increased risk of non-adherence to treatment and loss to follow-up, which can have serious consequences in terms of morbidity."
