A review has slammed the role health bodies played in Baby P's death. Neil Puffett investigates the changes that must be made.

Failure to share information, a lack of understanding and poor communication - these issues emerged as key themes in last week's Care Quality Commission report on the role health bodies played in the case of Baby P.

The review found that while health professionals had 34 contacts with Baby P in the lead up to his death, communication between different health professionals was not up to scratch. Interaction between health and social services was also found to be poor.

Laying the blame on inappropriate systems, as well as staff not adhering to processes, the document reveals shortcomings in communication were a major factor behind the inadequate assessment of needs and the subsequent lack of action.
