
The publication in late summer last year of the serious case review (SCR) into the death of four-year-old Daniel Pelka in 2012 led, along with SCRs into the deaths of Keanu Williams and Hamzah Khan published around the same time, to renewed high-profile public debates about child protection in England.
Coventry Local Safeguarding Children Board (LSCB) has now published what is termed a "retrospective deeper analysis and progress report" into the death of Daniel. This new review was completed following intervention from children's minister Edward Timpson who requested further evaluation of some "why" questions (about information sharing and recording, and social care assessments).
