Criticism of Pelka case review adds to concerns over investigation process

The review into the death of Daniel Pelka left many questions unanswered, raising concerns over the need to reform how serious case reviews are produced in order to address the root causes of failures.

Much of the serious case review (SCR) into the tragic life and death of four-year-old Daniel Pelka was sadly familiar.

The report, published last month by Coventry's Local Safeguarding Children Board (LSCB), highlights a host of missed opportunities by social workers, teaching staff, health professionals and the police.

But while no individual is explicitly blamed for failing to pick up on what was happening, the review has been criticised for failing to get to the bottom of why opportunities were missed and what can be done to prevent the same happening again.

Criticism

Prior to publication, Coventry's LSCB told CYP Now that the review would be conducted using a mixed methodology, drawing on aspects of the new "systems approach" advocated in the review of child protection following the death of Baby Peter Connelly, alongside traditional techniques.
