A source from the committee told Children Now that the report would highlight "management failures across the board" and "staff struggling to cope with the poor performance of a new IT system".
Another source said the report showed that the future of the organisation was "hanging in the balance".
Kate Green, chief executive of the Child Poverty Action Group, said children were the main losers from the continuing trouble at the agency.
