Local safeguarding children boards (LSCBs) carried out 302 serious case reviews between July 2007 and September 2009.
According to statistics released today, Ofsted rated 95 of these reviews as inadequate. Only 80 were considered good.
LSCBs, which oversee the work of social, education, health and other agencies involved in child welfare, conduct serious case reviews whenever a child dies and abuse or neglect are suspected to be a factor.
Ofsted in turn automatically carries out its own review of these inquiries.
In its 2008 annual report, Ofsted said even those reports rated as good took too long to produce.
