Ofsted's inspection report found a number of areas where priority action is needed to address poor record-keeping and delays in assessments.
This latest report comes a year after the council managed to reverse an "inadequate" rating for its child protection services, which had been handed down by Ofsted in 2007.
Inspectors who visited in August found "significant delays" in completing initial and core assessments, and the council's strategies were found to be "not sufficiently effective in managing and prioritising risk."
From case records inspectors were unable to confirm that all children concerned were safe.
In the cases of five children, the record-keeping was so poor that inspectors were unable to establish "that action had been taken to ensure that they were not at risk of harm."
