Jill Baker, director of Salford's children's services, has been suspended pending an investigation after the council was found to have problems with poor record-keeping and delays in assessments.
A number of areas requiring "priority action" were identified in an unannounced inspection conducted in August, during which Ofsted inspectors were unable to judge from case records whether all children concerned were safe.
In the cases of five children, the record-keeping was so poor that inspectors were unable to establish "that action had been taken to ensure that they were not at risk of harm".
