The report outlines concerns that early visits to the GP by Baby Peter’s mother about ailments and injuries sustained at home were not communicated or referred to other professionals including the health visitor.
"The threshold of concern at this point was the vulnerability of the child, and should have led to consideration of the need for a CAF [Common Assessment Framework] to be undertaken," the report states.
