The act can be based on music, dance or drama. It could carry a social message or be purely for entertainment.
The selected act will perform at the ceremony on the evening of Thursday 26 November at the Hurlingham Club in Fulham, west London in front of around 700 people from across the children's and youth sector.
The performance on the night will need to last no longer than four minutes. In order to be considered, send us a clip of the act no later than 18 September.
We will then invite up to four acts to London during the October half term to audition live in front of a small team of judges from CYP Now.
