Anne Owers has said Ashfield Young Offender Institution is neither directly managed by the regional NOMS representative nor directly line-managed by the Youth Justice Board, so it is not part of the regional framework that supports young offenders when they are released.
Owers said this weakness has potentially damaging long-term consequences and "needs urgent attention".
Serco, which runs Ashfield, said the issues around resettlement were being investigated.
