Letters: Case review blame game

Could I have read the front-page article about full disclosure of past tragedies right? (CYP Now, 8-14 June).

The purpose of serious case reviews, published or not, was for learning only and not to apportion blame. Yet Tim Loughton is quoted as saying: "When you read some of these reviews it becomes clear that a lot of fault can be laid at the door of other agencies."

Please tell me this is a misquote and that Loughton didn't really imply it's okay to have a go at other professionals or to ensure that fault is apportioned not just to social workers, with a tool that was supposed to enable lessons to be learned, as he himself reaffirmed in his letter to directors of children's services and local safeguarding children board chairs on 10 June.
