The Imam Muhammad Zakariya School in Dundee was severely criticised by inspectors more than a year ago. A follow-up report published last week said the all-girl independent school had still not done enough.
The school was ordered to do more to "broaden the experiences of pupils to prepare them better to play their part as Muslims in a culturally diverse Scotland".
Education minister Peter Peacock gave the school three months to demonstrate clear improvements.
