The results of a survey, carried out between January and March last year, show that out of 93 boards, only 17 reported their joint agency protocol was successful. A further 22 said it was working in part.
Although the report said that "considerable progress" has been made in developing child death review procedures since the last survey in 2006, "concerning" weaknesses were identified.
The survey also highlighted difficulty in determining whether or not the deaths were avoidable.
