Aisling Murray, 23, was sentenced to life after she admitted stabbing to death her daughter Chloe Fahey last May. The mother had repeatedly received hospital treatment for mental illness.
The review by Mike Cooney, chair of the Trafford Area Child Protection Committee, expressed surprise that children's services were only intermittently involved and that despite the mother's long stretches in hospital no joint planning between health and social services was ever considered. "Poor collaboration and communication between virtually all players is a major theme," he said.
Trafford Council said its social services department was considering the review's findings and would work to implement the 17 recommendations.
