Inspectors said assessments of the risk of harm offenders posed to others were not completed on time in a third of cases and were insufficient in around three in 10.
They also found Wirral's assessment of the likelihood of reoffending was insufficient in nearly half of all cases, and in just over half of cases offenders' vulnerabilities, such as excessive drinking, were not recorded in safeguarding assessments.
The offending service is to come up with an action plan to meet inspectors' recommendations in four weeks. Recommendations included a focus on regular, timely reviews and a more specific approach to what will be done to safeguard young people and minimise risk to others.
