The move allows the trust to focus on offering core social care and education services to young people in need in the north of England and North Wales.
AMNW chief executive Norman Goodwin said: "Adoption Matters has been providing a comprehensive adoption service in the region for over 60 years. This experience means we can provide an exceptional level of support for Together Trust adopters, and make changing to a new service provider as simple as possible."
Pat Walton, director of social care at the Together Trust, added that as well as giving the trust's current adopters access to specialist support from a dedicated agency, the transfer enables it to concentrate on maintaining and improving its primary services for young people and adults.
