According to a serious case review into the incident published this week, the 11-year-old known as Child X was in contact with a number of agencies including police, health, education and children’s services for a number of years over concerns about his behaviour.
But the review said inter-agency working was poorly planned and that the children’s social care services failed in their statutory duty to safeguard Child X. When children and adolescent mental health services raised concerns the review said they were not listened to or acted upon.
Director of children’s services Keith Moore said: "We fully accept the findings of the serious case review. We also accept that the failure of children’s social care to respond to the two letters from the child psychiatrist should not have occurred.
