
A child safeguarding practice review following the death of Isabella Jonas-Wheildon found that chances to protect her were marred by ‘siloed decision making’.
Report author Russell Wate said no multi-agency discussions took place in her case, despite police, social worker and health involvement in her families’ lives, particularly in the weeks leading up to her death.
He said that Isabella had become “invisible” to authorities involved in her case.
Isabella was murdered in June 2023 by her mother's partner Scott Jeff, 24, who was jailed for life in December 2024 and ordered to serve a minimum term of 26 years, following a trial.
