The toolkit, which contains case studies, guidance and useful facts, is part of the government's Link Up, Link In campaign, which aims to improve joint working and communications between different public services.
It will help professionals support care leavers, offenders under probation supervision, people with a learning disability and people with mental health problems.
Angela Smith, minister for the third sector and social exclusion, said the government is keen to get more vulnerable people into a stable home and sustainable employment.
