Sarah McCarthy-Fry, minister for schools and learners, unveiled the Aiming High for Disabled Children Transition Support Programme for disabled young people aged 14 to19 yesterday (2 December).
Every local authority area is receiving £10,000 to assess their current support for transition including gathering the views of young people and families and completing a self-assessment questionnaire.
Thirteen authorities are to receive an extra £37,500 to extend their work on engaging disabled young people and their families in education, employment and training options for young people over 16.
McCarthy-Fry said that a national transition support team, a coalition led by the Council for Disabled Children, was "well positioned" to work with the key local and regional agencies to help make the changes that will make a difference to disabled young people.
The government will invest £19m over three years in transition services through the programme.
