Andrew McLauchlan faces claims he did not make sufficiently frequent visits to children on the child protection register.
It is also alleged McLauchlan, who worked for East Sussex County Council between November 2001 and February 2006, failed to maintain a reasonable standard of communication with other professionals in the cases of four children.
He will face a total of 13 allegations at a General Social Care Council (GSCC) hearing due to begin on Tuesday 4 August.
The hearing is scheduled to last four days.
A spokesman for East Sussex County Council said: "It is a confidential internal personnel issue but we can confirm that in February 2006, following established disciplinary procedures, Andrew McLauchlan's employment for East Sussex County Council was terminated.
"In accordance with the code of practice for employers of social care workers we informed the GSCC of our internal investigation and its conclusion."
