Ofsted inspectors reported that practitioners or their managers did not respond appropriately in five cases where risks had been identified, or alleged, including domestic violence.
The inspection report states: "While none indicated that a child was at immediate risk of significant harm, there was sufficient concern for Ofsted to refer these cases to the head of service, requesting an urgent review."
Despite this, the report also said some aspects of the service delivery in the area were "strong", with the organisation meeting most of its key performance indicators and exceeding some.
Cafcass chief executive Anthony Douglas said: "The inadequacies in our safeguarding practice, while not placing any individual child at risk, are unacceptable. Immediate steps were taken to ensure full compliance with our policies and procedures.
