Inspectors said the most serious failing was a lack of consistency in looking after the interests of some young people. Other failings included the service focusing too much on processes instead of outcomes and concerns over the quality of practitioners' work with children and families.
Ofsted found some case files were inadequate and had significant omissions.
Cafcass chief executive Anthony Douglas said it was putting a new structure in place to improve the quality of its work.
