This publication was launched in May 2009, and will be welcomed by all families and professionals who support children and young people with life-limiting and life-threatening conditions. One of the main aims of the guide is to provide families and carers with an accessible and user-friendly resource that mirrors the ACT Children's Palliative Care Pathway (2004).
This unique step-by-step guide helps families through the complex and often confusing processes that they will go through and explains what they should expect in terms of services, contact with professionals and other agencies along that journey. For ease of reference, the five main sections are colour-coded to mirror the ACT Care Pathway standards, and explain how to apply the standards in the context of the whole family.
