Transition support for disabled young

Plans must be drawn up for all disabled young people moving from child to adult services, according to government guidance.

A good practice guide released last week states that health and other professionals working with disabled children approaching adulthood should put together plans detailing the workers who are involved, the support the young person needs and when they will get it.

The document, A Transition Guide for All Services, also outlines how to plan the shift from child and adult services for young people with complex health needs.

The guidance says children with life-threatening conditions are increasingly reaching adulthood and, as a result, adult health services are having to treat conditions they are unfamiliar with. It says transition planning can help ensure different specialists work together to support these young people when they turn 18.
