Simon Davies, chief executive of the Teenage Cancer Trust, said: "Hospitals do not have to support education post-16. So if you don't have a supportive school or someone in the health system who is going to help you, you could be in trouble."
Davies said the trust is planning to develop a system of hospital-based education mentors to bridge the gap between the two sectors.
